In this case there is a doubt about the preoperative scotopic pupil size of the patient. The author reports a scotopic pupil size greater than 7.0 mm. (An exclusion criterion for the Artisan pIOL is a pupil larger than 7.0 mm.) I wonder what the pupil size was postoperatively. If a Seidel test is positive, it proves wound leakage.

Kohnen et al.1 report a case of intermittent myopic shift after implantation of an Artisan pIOL. They accurately describe the symptoms, possible examinations, and solutions. Güell reported a similar case. I have also observed this phenomenon (unpublished case). After changing the site of enclavation, the phenomenon of spasm of accommodation resolved. Because of the relationship between pupil size and accommodation, I suggest changing the site of enclavation.
